Trillium Advantage Dual (HMO SNP)

If you qualify for Medicaid coverage through the state of Oregon and you are eligible for Medicare, you can enroll in our Dual Eligible Special Needs Plan (D-SNP).

The Trillium Advantage Dual (HMO SNP) plan was created to combine coverage for your Medicare and Medicaid benefits PLUS give you extra benefits under one plan and one insurance company. With the Trillium Advantage (HMO SNP) plan, you will be able to connect with us for all of your health care needs. This means you'll have:

Extra Benefits for No Additional Premium

Trillium Advantage Dual (HMO SNP) includes coverage for a routine eye exam and an allowance for supplemental eyewear. You also get a gym membership for no additional cost. If you are eligible for Medicare and are living in an institution such as a nursing home or long-term care facility, you should consider the Trillium Advantage TLC ISNP (HMO SNP) plan. This is an Institutional Special Needs Plans (ISNP) that gives you all the benefits of Medicare parts A and B, along with the extra benefits of a Medicare Advantage plan and the TLC we all need.

If you are eligible for Medicare and receiving home health care services equal to those of a nursing home, you should consider the Trillium Advantage TLC Community ISNP (HMO SNP) plan. This is an Institutional Special Needs Plans (ISNP) that gives you all the benefits of Medicare parts A and B, along with the extra benefits of a Medicare Advantage plan and the TLC we all need.

"Extra Help" and Low Income Subsidy Information

The information below applies to all three of the plans listed above:

Medicare's "Extra Help" Program

Medicare provides "Extra Help" to pay prescription drug costs for people who have limited income and resources. Resources include your savings and stocks, but not your home or car. If you qualify, you get help paying for any Medicare drug plan's monthly premium, yearly deductible, and prescription co-payments. This Extra Help also counts toward your out-of-pocket costs.

Some people automatically qualify for Extra Help and don't need to apply. Medicare mails a letter to people who automatically qualify for Extra Help.

To see if you qualify for getting Extra Help, call:

  • 1-800-MEDICARE (1-800-633-4227). TTY users call 1-877-486-2048, 24/7, and see www.medicare.gov 'Programs for People with Limited Income and Resources' in the publication Medicare and You; or
  • The Social Security Administration at 1-800-772-1213 between 7:00 a.m. and 7:00 p.m., Monday through Friday. TTY users call 1-800-325-0778; or
  • Your State Medicaid Office

After you apply, you will get a letter letting you know if you qualify for Extra Help and what you need to do next.

Low Income Subsidy Premium Benefit

If you qualify for a Low Income Subsidy, you do not pay a separate monthly plan premium for our plan. While we have a monthly premium of $34.80 per month, Medicare pays this on your behalf because you are eligible for both Medicare and Extra Help.

You must continue to pay your Medicare Part B premium.  However, as a dual eligible (have Medicare and Medicaid), the state of Oregon will cover your Part B premium as long as you retain your Medicaid eligibility.
